Have you tried the shuttle in other programs at all?
I have 2 USB3 UltraStudio SDI and I have to change drivers depending on the program I want to use (touch designer, premier, resolume). Every program crashes at different times with different drivers, just have to find one that works with your hardware.
Read the readme notes of the drivers and find out the last time they updated the shuttle itself.
When BMD give you an "update" they do it most of the time to support new hardware, but breaks existing stuff.
Maybe they should just do away with the all in one DV driver and update specific hardware
Like Joris mentioned, tried rolling back drivers to find one that works.

Re: BSOD with BlackMagic Design Intensity USB 3 & Thunderbol
Hey!
So, gone back to the drivers suggested earlier and also now running resolume in windows xp compatibility mode.
Seen a marked improvement, had a few crashes when it loads but not sure if they are related. Encoded all my videos to DXV as well. I was also in the process of installing vvvv (for different reasons) and it said that my directx was out of date which was weird! I updated that and since then it (touch wood) seems to be working!
Real test will be this weekend and the gig I have... It does seem to be working! Any other suggestions to check for or troubleshooting ideas?
