Polygon stretching points not working as expected.

I have a project where I want to sample an image, in perspective, then corner-pin those areas onto a square surface.
However, the stretch is pinching across odd areas instead of an even stretch. I was trying to pick 4 points of the input, then square those up on the outputs.. but that's not working.
Is there a way to do this? My current input/output screens are attached.
