Puzzle overlay bug

Resolume on a mac at the church I work at has a problem where when it outputs to the 3 outputs (2 projectors and a LED wall) sometimes it will do a puzzle pattern overlay over the current picture. It will only go away after we restart the program. Can anyone help?

How are you outputs configured? Layer/group direct to an output, or same thing on all outputs?

I dont know how they are setup but I think its layer/group. The middle screen is a different input then the two side screens, the two side screens always show the same thing.

How do I fix this weird overlay that sometimes appears randomly?

If you create a new composition when the pattern shows up, does it go away?
Does it go away if you reload your current composition?
