Right before this happened, I'd created a new composition, dragged in 6 images, and then begun resizing them to the composition using my shortcut <Ctrl+Alt+S>.

BUG: Wrong clip being sent to output
This is very odd, but I'm sure I've seen if before. The layer contains one image, but a different one is being sent to the main output. See image below:
Right before this happened, I'd created a new composition, dragged in 6 images, and then begun resizing them to the composition using my shortcut <Ctrl+Alt+S>. UPDATE: Sometimes it does this - see below. The larger image is the one that was selected immediately beforehand.
UPDATE 2: There's something definitely not right here. I created another new composition, dragged in the same images, and it's happening now with a different slot in the deck.
