I'm using the same setup as you.
Output 1 is HDMI via HDMI port.
Output 2 is coming out of my TB3 to two HDMI port adapter.
In theory, With that adapter, I should be able to get a 3rd HDMI output, but the firmware only allows for 1 HDMI output via the TB3 port. They are working on an update that will allow for a 3rd total output. (of course you can use Decklinks or Trippleheads to split outputs as well)

I had to return my 1060 blade and now I've got a 970m blade, but I can't get the 2 outputs at the same time like you guys did. Does anybody have an idea if it's the 970m version problem or if it's the adapter problem? And one more annoying issue, as this is the 970m version, the main screen is a QHD resolution, but this resolution is giving me a huge problem with the second display output, no matter what I do, my Resolume NEVER sees the right resolution, it just upscales it, I have already tried leaving the scale on 100% on both the main and the second display, I've tried setting the main display resolution to 1080p but nothing seems to solve this.

I'd contact Razer on this one.
Is your adapter TB3 or USB 3.1? Does your 970m mobo have TB3 or USB 3.1?
The adapter will be ran through Windows (Intel Chipset), even though the Nvidia card will be doing the processing, so make sure to configure your second output through the Windows settings, and your first output will be configured through the nvidia control panel (or windows settings if you prefer)
